  • Needle stick and sharp injuries
  • An employee who independently and voluntarily requests that his or her name not be entered on the OSHA 300 log1.
  • This application supports privacy Concern Cases.

This Application does support Privacy Concern Cases but their information should be documented on a separate confidential and secure list as per OSHA regulations.
